The Indiana State Department of Health conducted a total of eight recertification surveys and of those, two (25%) were found to be deficiency free. There were a total of 14 Offense tags cited, 44 Deficiency tags cited for a total of 58 Residential tags for the month of June.

The Offense tags are as follows:

Tag 241 (7 times) – Health Services Offense for failure to ensure the administration of medications and the provision of residential nursing care is as ordered by the physician and supervised by a licensed nurse on the premises.

Tag 052 (5 times) – Residents’ Rights Offense for failure to ensure the right of each resident to be free from abuse of any kind.

Tag 051 (1 time) – Residents’ Rights for failure to ensure the right if each resident to be free from any physical or chemical restraint imposed for the purposes or convenience and not required to treat the resident’s medical symptoms.

Tag 406 (1 time) – Infection Control Offense for failure to establish and maintain an infection control practice designed to provide a safe, sanitary, and comfortable environment and to help prevent the development and transmission of diseases and infection.

The top five Deficiency tags are as follows:

Tag 273 (8 times) – Food and Nutrition Services

Tag 144 (7 times) – Sanitation and Safety Standards

Tag 117 (3 times) – Personnel

Tag 217 (3 times) – Evaluation

Tag 298 (3 times) – Pharmaceutical
