The Indiana State Department of Health has issued an updated spreadsheet and corresponding instructions for long-term care facilities to enter the COVID-19 data required to be submitted by July 14, 2020 at 6 PM EDT. The previously issued spreadsheet asked for dates to be entered as MM-DD-YYYY but was issuing error messages. The updated spreadsheet asks for dates to be entered as MM/DD/YYYY and is now functional.

As a reminder, all licensed and unlicensed long-term care facilities, including assisted living communities, are required to submit the completed spreadsheet here by July 14th at 6 PM EDT, even if a facility has had no COVID-19 cases.
