Vaccination Survey for Long-Term Care Facilities

The Indiana Department of Health (IDH) has requested that all comprehensive and residential care facilities (i.e., nursing facilities and licensed assisted living) complete this online survey by close of business Wednesday, March 17, 2021.  The purpose of the survey is to help IDH better understand vaccination rates across the state and identify ways to support facilities as they help vaccinate residents and staff.

IDH stated that Probari will follow-up with those facilities who do not complete a survey by the deadline.  Please submit only one survey response per facility and reach out to Probari at outreach@probarisystems.com or (317) 804-4102 with any questions.
