The Indiana State Department of Health cited providers (4) with the following immediate jeopardy citations as follows:

F880 (3 times) – Infection Prevention & Control

  • Failure to ensure infection control practice for COVID-19 were followed to ensure residents free from symptoms of the COVID-19 infection, on the dementia care unit, were not exposed to and roomed with residents who were positive for the COVID-19 infection.
  • Failure to follow CDC guidelines during a pandemic and implement an infection control program for timely and accurate assessment of signs and symptoms of COVID-19.
  • Failure to follow infection control procedures to protect resident during the COVID-19 outbreak. Failure implement isolation procedures and have PPE accessible in the resident rooms

F624 (1 time) – Preparation for Safe/Orderly Transfer/Discharge

  • Failure to ensure a safe and orderly transfer during the COVID-19 Pandemic.
