New RedCap Survey for COVID-19 Reporting – Effective Monday, April 20, 2020

Pursuant to the State Health Commissioner Order issued on April 8, 2020 long term care facilities are required to report within 24 hours to the Indiana State Department of Heath all positive COVID-19 test results for residents and employees, and all deaths related to or suspected to be due to COVID-19 for residents and employees.  ISDH has developed a survey tool, called the RedCap Survey, to collect this data.

As of Monday, April 20, 2020, ISDH  has updated the RedCaps survey  and long-term care facilities reporting COVID-19 cases and deaths must use this new link, which can be found here.   Please note that any previous links will not work.

The new RedCap Survey will allow long-term care facilities to enter their facility demographics one time.  Long-term care facilities will then only need to add the client record information for their facility when reporting subsequently.

As a reminder, long-term care facilities are required to report the following within twenty-four (24) hours:

  • Any resident who tests positive for COVID-19
  • Any employee who tests positive for COVID-19
  • Any confirmed positive COVID-19 related death OR suspected COVID-19 related death of a resident
  • Any confirmed positive COVID-19 related death OR suspected COVID-19 related death of an employee

Confirmed or suspected deaths should be reported regardless of where the death occurred and within twenty-four (24) hours of a facility’s knowledge of the death.  The entirety of the Indiana State Department of Health’s (ISDH) notice on the new RedCap Survey can be found here.
