The Division of Aging has updated our email addresses with intent to better serve all inquiries more efficiently. Going forward please direct inquires to the most appropriate email listed below.
- Interest in becoming an Aged & Disabled Waiver, Traumatic Brain Injury Waiver or Money Follows the Person provider.
- Applications may be submitted to the Division of Aging provider applications email: Daproviderapp@fssa.IN.gov
- Information about becoming a waiver provider may be found on the Division of Aging webpage, by clicking here.
- Active Aged & Disabled Waiver, Traumatic Brain Injury Waiver or Money Follows the Person providers needing to update your profile.
- This may include but not limited to adding or removing services, add or remove counties to serve, change of ownership, or address changes.
