The Division of Aging has updated our email addresses with intent to better serve all inquiries more efficiently. Going forward please direct inquires to the most appropriate email listed below.

  • Interest in becoming an Aged & Disabled Waiver, Traumatic Brain Injury Waiver or Money Follows the Person provider.
    • Applications may be submitted to the Division of Aging provider applications email: Daproviderapp@fssa.IN.gov
    • Information about becoming a waiver provider may be found on the Division of Aging webpage, by clicking here. 
  • Active Aged & Disabled Waiver, Traumatic Brain Injury Waiver or Money Follows the Person providers needing to update your profile.
    • This may include but not limited to adding or removing services, add or remove counties to serve, change of ownership, or address changes.
    • Send requests to: Daproviderapp@fssa.IN.gov
  • Provider payment issues Aged & Disabled Waiver, Traumatic Brain Injury Waiver or Money Follows the Person
    • Submit a CaMSS Help Desk Ticket, by clicking here.
    • **Please do NOT email the Division of Aging directly.  When a ticket is submitted it will be assigned, researched and responded through this ticket system**
