IHCA/INCAL’s Senior Living Award program recognizes professionals in nursing facilities and assisted living communities across the state of Indiana for their exemplary contribution and unwavering commitment to delivering quality care.

Nomination Requirements for Best of Indiana – Long Term Care Facility

This category is designed to recognize the best of Indiana’s long term care facilities and to educate community leaders about our profession.  This awards program addresses the loving and compassionate way that a facility’s staff cares for their residents and what the facility is doing to continue to provide quality care and meet residents’ needs.

  • Each member facility may only submit one nomination form.
  • Designated Contact: Each member facility must choose one person to serve as the designated contact.  All follow up correspondence, award information, nomination questions, etc will be sent to the Designated Contact.
  • All submissions should be completed online by the deadline.
  • IHCA/INCAL member in good-standing
  • Facilities must be in good-standing with ISDH/CMS and regulatory compliance
  • Please provide specific examples and describe what makes your facility deserving to be the Best of Indiana:
    • Facility’s mission statement/operating philosophy and what unique programs/activities the facility conducts in order to support this mission/philosophy
    • Identify and describe creative and innovative interventions that address common long term care challenges and share best practices which have promoted and enhanced quality of care
    • Describe your facility performance/excellence and commitment to quality care.  Define how your facility goes above and beyond.
    • Supports the greater community

Nomination Requirements for Best of Indiana – Assisted Living Community

This category is designed to recognize the best of Indiana’s assisted living communities and to educate community leaders about our profession.  This awards program addresses the loving and compassionate way that an assisted living community’s staff cares for their residents and what the facility is doing to continue to provide quality care and meet residents’ needs.

  • Each member facility may only submit one nomination form.
  • Designated Contact: Each member facility must choose one person to serve as the designated contact.  All follow up correspondence, award information, nomination questions, etc will be sent to the Designated Contact.
  • All submissions should be completed online by the deadline.
  • IHCA/INCAL member in good-standing
  • Facilities must be in good-standing with ISDH and regulatory compliance
  • Please provide specific examples and describe what makes your facility deserving to be the Best of Indiana:
    • Facility’s mission statement/operating philosophy and what unique programs/activities the facility conducts in order to support this mission/philosophy
    • Share best practices that are a result of an interdisciplinary effort and promote creative innovations and wellness programming
    • Facility performance/excellence and commitment to enhancing quality of life.  Define how your facility goes above and beyond.
    • Supports the greater community

Nomination Requirements for Nurse of the Year, C.N.A. of the Year and Caregiver of the Year Award

  • All submissions should be completed online by the deadline.
  • IHCA/INCAL member facility in good-standing
  • Facilities must be in good-standing with ISDH/CMS and regulatory compliance
  • Please provide specific examples of how the person has excelled in EACH of the following areas:
    • Job performance/Excellence/Personal demeanor
    • Commitment to quality care and quality of life, delivers person-centered care
    • Serves as a role model and mentor to their peers, teamwork/co-worker interactions and communications
    • Supports the facility or greater community
    • Exhibits leadership and supervisory skills (if applicable)

Additional Information

  • Each nomination must exclude any information that would identify the facility or individual to a Panel Reviewer, such as facility name, facility location, individual and staff names.
  • Chosen award recipients will be asked to participate in an award video shoot and/or may be asked to submit a photo for use of award recognition
  • Nominees must not be a previous Senior Living Award recipient
  • While several individuals might nominate the same facility/individual, the free convention registration is limited to only one winner and one nominator
  • Winners will be chosen by a panel of judges selected by IHCA/INCAL.  IHCA/INCAL reserves the right to not present any award.
