Help IHCA recognize programs, staff, and commitment to quality care by submitting nominations for the 2010 Long Term Care Awards! Innovative people and programs in geriatric settings, including Skilled Nursing, Assisted Living, Adult Day Care, Adult Foster Care, Memory Care, Independent Living, Senior Centers, and Retirement Centers, are essential in delivering quality care to our seniors.  All winners will be announced at the Long Term Care Convention on Wednesday, May 26, 2010 at the Quality Award Reception at Lucas Oil Stadium.

Employee Awards

Take a few minutes to show your appreciation for staff and colleagues who go above and beyond to deliver quality care by nominating them for an Employee of the Year Award.

 

Award Categories:

Therapist          Medical Director
Support Staff    Human Resource/Financial Professional
Dietitian          Certified Nursing Assistant
Nurses

Nomination Requirements
• All submissions should include a typed or neatly printed nomination form
• Please provide specific examples of how the employee has excelled in EACH of the following areas:
o job performance
o employee interaction/teamwork
o commitment to quality care/quality of life
o community involvement

Executive of the Year    

If your Long Term Care Executive embodies caring leadership and a commitment to staff, residents, and the community nominate him or her for the Executive of the Year Award.

Nomination Requirements
• All submissions should include a typed or neatly printed nomination form
• Two letters are needed from colleagues stating why the executive should be selected as Executive of the Year (one page limit per letter).
• Please provide (either typed or handwritten) specific examples of how the Executive has excelled in EACH of the following four areas:

o facility and/or program enhancement
o community involvement
o staff development
o resident/family relations

Social/Activity Program of the Year
Mental well-being is just as important as physical well-being.  Nominate individuals who have developed innovative programs that meet the needs and interests of residents in geriatric settings. 

Nomination Requirements
• All submissions should include a typed or neatly printed nomination form
• Please provide (either typed or handwritten) specific examples of how the employee has excelled in EACH of the following areas:

o community involvement
o commitment to meaningful and stimulating activities
o commitment to quality care/quality of life

Corporate Stewardship Award

The Corporate Stewardship Award showcases businesses and not-for-profits for demonstrating ethical leadership and corporate stewardship and for making a positive difference in society.

Nomination Requirements
• All submissions should include a typed or neatly printed nomination form
• Please provide (either typed or handwritten) specific examples in the following areas:

o Tell the Story: Please give a brief description of your stewardship - what you are doing, what have you achieved and how have you made a difference.

o Business Strategy: Describe how the program supports the overall business strategy for your company.  

o Evaluation and Impact: Identify the principal immediate and long term social and economic benefits and the metrics used to evaluate them.

Excellence through Education
The Excellence through Education Award recognizes individuals advancing their career in the long term care field through continuing education.  Individuals who have shown a desire to further their skills and have shown a devotion to caring for those needing long term care are eligible for this award. 

Nomination Requirements
• All submissions should include a typed or neatly printed nomination form
• Submissions should include documentation showing previous position held,
transcripts from accredited university or college showing completion of an
associate's, bachelors or master's degrees, and new position.
• These awards may be self-nominated
• Please provide (either typed or handwritten) specific examples in the
following areas:

o Tell the Story: Please give a brief description of your commitment to working in long term care and continuing your education.
o Dedication to quality care
o Dedication to education

Volunteer of the Year

Volunteers are exceptional people who give the greatest gift - time.  Recognize those who devote their loving time to brighten the lives of those in geriatric settings.

Nomination Requirements
• All submissions should include a typed or neatly printed nomination form
• One letter is needed from a staff member or resident, outlining why the volunteer should be selected for the award (one page limit).
• Submit how frequently the nominee volunteers and approximately how many hours the nominee volunteered.
•  Please provide specific examples of how the volunteer has excelled in the following categories:

o resident interaction
o community involvement
o resident/family relations
